Low Testosterone (Male Hypogonadism) Treatment in Indore

Testosterone is the principal male hormone. It governs sexual desire, erectile quality, sperm production, muscle mass, bone density, red blood cell production, mood and energy. When levels fall below normal, the effects show up across all of these systems at once, which is why men with low testosterone often describe a vague sense that “everything has slowed down” rather than a single specific complaint.

What is Low Testosterone?

Male hypogonadism is diagnosed when total testosterone measured in the early morning is persistently below the reference range on at least two occasions, together with matching symptoms. Primary hypogonadism arises from the testes themselves; secondary hypogonadism arises from the pituitary or hypothalamus failing to stimulate them. Distinguishing between the two changes the treatment completely, particularly for men who still want children.

Causes of Low Testosterone

1. Testicular causes: Undescended testes, mumps orchitis, testicular trauma, varicocele, chemotherapy or radiation, and Klinefelter syndrome.
2. Pituitary and hypothalamic causes: Pituitary tumours, high prolactin, head injury and certain genetic conditions.
3. Metabolic causes: Obesity, type 2 diabetes, metabolic syndrome and obstructive sleep apnoea. This is by far the largest group in Indian clinics.
4. Drug-induced: Anabolic steroid abuse in gyms, long-term opioids, glucocorticoids and some antifungals.
5. Chronic disease: Kidney failure, liver cirrhosis, HIV and any severe chronic illness.
6. Ageing: A gradual, modest decline of roughly 1 percent per year after 30, which alone rarely explains marked symptoms.

Symptoms of Low Testosterone

1. Sexual: Reduced desire, fewer morning erections, erectile difficulty and reduced ejaculate volume
2. Physical: Loss of muscle bulk, increased abdominal fat, reduced body and facial hair, gynaecomastia
3. Energy and mood: Persistent fatigue, low mood, irritability and poor concentration
4. Skeletal: Reduced bone density, and fractures from minor trauma in long-standing cases
5. Fertility: Low sperm count and difficulty conceiving

When to See a Doctor

Get tested if you have persistent fatigue with reduced sexual desire, loss of morning erections, unexplained loss of muscle mass, infertility, or if you have type 2 diabetes or significant obesity. Men who have used anabolic steroids should be assessed even after stopping, as suppression can persist for a long time. Very low testosterone with headache or visual field problems needs prompt pituitary evaluation.

Treatment for Low Testosterone

1. Confirm the diagnosis properly: Two separate early-morning fasting samples, plus LH, FSH, prolactin and a full metabolic panel to determine the type.
2. Treat reversible causes first: Weight reduction, sleep apnoea treatment, diabetes control and stopping offending drugs can normalise levels without any hormone therapy.
3. Correct a varicocele where present, which can improve both testosterone and sperm parameters.
4. Testosterone replacement therapy: Injections, gels or long-acting preparations, under supervision, with monitoring of haematocrit, PSA and lipids.
5. Fertility-sparing therapy: For men who want children, hCG or clomiphene is used instead of testosterone, because testosterone replacement suppresses sperm production.

What Happens If You Delay Treatment

Untreated hypogonadism is not simply a sexual inconvenience. Bone density falls silently for years until a fracture reveals it, muscle is replaced by visceral fat which worsens insulin resistance, and the resulting metabolic decline further suppresses testosterone in a self-reinforcing loop. Fertility also declines. Testing is a single blood sample and treatment, when needed, is straightforward and effective.

Frequently Asked Questions (FAQs)

When should the blood sample be taken?

Between 7 and 11 am in a fasting state, because testosterone follows a daily rhythm and afternoon samples read falsely low.

Does testosterone therapy cause infertility?

Yes. External testosterone suppresses the body's own sperm production. Men planning children are treated with hCG or clomiphene instead.

Can weight loss raise testosterone?

Substantially. Fat tissue converts testosterone to oestrogen, so meaningful weight loss often restores levels without hormone therapy.

Is testosterone therapy safe for the prostate?

It is not given in known prostate cancer. With baseline and periodic PSA monitoring, supervised therapy is considered safe in appropriately selected men.

How soon will I feel better?

Sexual desire and mood usually improve in 3 to 6 weeks; muscle and bone changes take several months.

Are gym supplements and boosters useful?

Over-the-counter boosters have no proven effect, and anabolic steroids sold in gyms actively cause the very deficiency they are marketed to treat.
