Itchy Scalp Treatment in Indore

Scalp itching is a symptom, not a diagnosis, and this distinction matters more than it might seem. Patients almost universally assume itching means dandruff and buy an anti-dandruff shampoo. When the itch is actually from fungal infection, psoriasis, lice or a hair dye allergy, that shampoo will not help, and months are lost. Identifying the cause takes one examination of the scalp.

Common Causes of an Itchy Scalp

5. Contact dermatitis: Allergic reaction to hair dye (particularly paraphenylenediamine), shampoos, serums or hair colour, causing itching, swelling and sometimes weeping.
6. Folliculitis: Bacterial infection of the hair follicles producing itchy, tender pustules.
7. Lichen planopilaris and discoid lupus: Scarring conditions causing itch, burning and permanent hair loss. These need early specialist treatment.
8. Dry scalp and irritation: From over-washing, hot water, harsh shampoos and hard water.

Clues That Point to the Cause

1. Greasy yellow flakes with redness suggests seborrhoeic dermatitis
2. Thick silvery plaques extending past the hairline suggests psoriasis
3. Patchy scaling with broken hairs, especially in a child, suggests tinea capitis
4. Itching at the nape and behind the ears with visible nits suggests lice
5. Itching starting within days of colouring the hair suggests contact allergy

When to See a Doctor

See a dermatologist if itching persists beyond a few weeks despite medicated shampoo, if there is hair loss, if the scalp is sore, weeping or has pustules, or if patches of scalp appear smooth and shiny, which suggests scarring. Scalp itching with swelling of the face after hair colouring should be treated urgently as an allergic reaction.

Treatment for Itchy Scalp

1. Correct diagnosis first: Clinical examination, dermoscopy of the scalp (trichoscopy), and KOH or fungal culture where infection is suspected.
2. Antifungal therapy: Medicated shampoos for seborrhoeic dermatitis; oral antifungals for tinea capitis, which cannot be cured topically.
3. Anti-inflammatory treatment: Topical steroid or calcineurin inhibitor lotions, and coal tar or salicylic acid preparations for psoriasis scale.
4. Lice treatment: Permethrin or ivermectin, repeated after 7 to 9 days to kill newly hatched lice, with simultaneous treatment of all affected household members and hot washing of linen.
5. Allergy management: Stopping the causative product, patch testing to identify the allergen, and using PPD-free alternatives.
6. Antibiotics: For bacterial folliculitis.
7. Scalp care: Lukewarm rather than hot water, gentle sulphate-free shampoo, avoiding overnight oiling on an inflamed scalp, and not scratching.
8. Address associated hair fall once the underlying scalp condition is controlled.

What Happens If You Delay Treatment

Persistent scratching damages hair shafts and follicles, causes lichenification and invites secondary bacterial infection. Untreated tinea capitis in children can progress to a kerion, a boggy inflammatory mass that causes permanent scarring hair loss. Most importantly, the scarring alopecias, lichen planopilaris and discoid lupus, destroy follicles irreversibly, and every month of delay means follicles that can never be recovered. An itchy scalp with hair loss should never be managed by trial and error with shampoos.

Is an itchy scalp always dandruff?

No. Fungal infection, psoriasis, lice and hair dye allergy all cause itching and need entirely different treatment. Examination is what distinguishes them.

Can anti-dandruff shampoo cure fungal scalp infection?

No. Tinea capitis lives within the hair follicle where shampoos cannot reach, and requires oral antifungal treatment.
