Aching, tired, heavy legs at the end of the day is one of the most commonly ignored symptoms in medicine. Patients attribute it to age, to their job, or to simply having been on their feet too long. Telling Venous, Arterial and Nerve Pain Apart

1. Venous pain: Dull, heavy aching that builds through the day, is worse after prolonged standing or sitting, and is relieved by walking, elevating the legs or a cold shower. Often accompanied by ankle swelling.
2. Arterial pain (claudication): Cramping calf pain that comes on after walking a predictable distance and is relieved by standing still. Worse walking uphill. Suggests peripheral arterial disease.

2. Occupations involving prolonged standing: teachers, surgeons, shopkeepers, security staff, hairdressers
3. Obesity and sedentary lifestyle
4. Pregnancy and hormonal contraceptives
5. Peripheral arterial disease, particularly in smokers and diabetics

Symptoms That Accompany It

1. Tiredness and aching in the calves and thighs by evening
2. Ankle swelling and sock marks
3. Visible thread veins or bulging varicose veins
4. Night cramps and restlessness in the legs
5. Itching or brown discolouration around the ankle in longstanding cases

See a vascular specialist if heaviness has persisted for weeks, is associated with visible veins, swelling or skin changes, or is interfering with work. Seek urgent assessment for sudden severe calf pain with swelling, for pain that appears at rest or at night in a smoker or diabetic, or for any non-healing wound on the leg or foot.

Treatment for Leg Pain and Heaviness

1. Venous Doppler ultrasound: The definitive test, showing exactly which veins have failed valves. Symptoms often precede visible veins, so a normal-looking leg does not rule venous disease out.
2. Compression stockings: Graduated class I or II stockings give substantial symptomatic relief and slow progression.
3. Venoactive medication: Micronised flavonoids and similar agents reduce heaviness and swelling.

What Happens If You Delay Treatment

Venous disease is progressive and follows a predictable sequence: heaviness, then visible veins, then ankle swelling, then skin pigmentation and hardening, and finally ulceration. Each stage is harder and slower to treat than the one before it. Treated at the heaviness or early varicose stage, a single daycare laser session usually resolves it. Treated at the ulcer stage, the same patient faces months of dressings with a high chance of recurrence.

Can I have venous disease without visible varicose veins?

Yes, and it is common. Heaviness and evening swelling often precede visible veins by years. A Doppler scan reveals the underlying reflux.

Why do my legs feel better when I walk?

Walking activates the calf muscle pump, which pushes venous blood back towards the heart. Relief on walking strongly suggests a venous cause.

Are compression stockings uncomfortable in Indian summers?

They take some getting used to, but modern breathable graduated stockings are far more tolerable than older designs and are worn only during the day.
